JOB SUMMARY: 

The Ruth Ellis Center (REC) is looking for someone with a passion for advocacy and housing justice. The Case Manager provides direct youth-centered and strengths-based case management to LGBTQ youth and young adults ages 18 to 30 in Ruth Ellis Center’s Housing Program for Survivors of Trafficking (HPST) and Drop-In Center Programs. The Case Manager (1) encourages engagement of youth, (2) supports youth to accomplish their goals around achieving stability, (3) gains a deep understanding of available local and regional resources to reduce and remove barriers to employment and housing, and (4) advocates for systems change. The Case Manager provides services both at the drop-in-center and in the community (i.e. coffee shops, home visits).

The Case Manager is responsible for supporting the attainment of individualized goals by assisting participants in accessing, retaining, and actualizing services, including safe and stable housing; education and employment; benefits; behavioral and primary care; and other support services.

 

ESSENTIAL FUNCTIONS:

  1. Carry a caseload of youth in the HPST and Drop-In Center programs, which requires:
    1. Building authentic, caring, and affirming relationships with REC youth.
    2. Participating in weekly case review meetings.
    3. Providing a trauma informed and anti-oppression approach to youth, colleagues, and self that reflects REC’s Principles of Work (trauma-informed care, positive youth development, harm reduction, and restorative justice).
    4. Developing self-care practices.
    5. Building and maintaining strong working relationships with other providers.
    6. Completing intakes and assessments with youth.
    7. Accompany clients to community resources such as apartment searching; applying for benefits; medical appointments; and other appointments to assist with enrollment and/or advocacy as needed.
    8. As directed by clients, assisting in applying for personal protection orders, filing reports with police, attending forensic examinations, and court advocacy/accompaniment.  
    9. Supporting youth in identifying their strengths and use them to set goals together
    10. Providing youth flexible, responsive case management that address their variety of needs including: accessing shelter; obtaining identification; enrolling in high school, GED programs, and college; navigating financial aid; accessing public benefits; connecting to health, mental health, and substance abuse care; accessing childcare; connecting to employment and permanent housing; and other supports.
    11. Checking in with youth at least one a month, and more frequently as needed, about progress on their goals and how REC and its partner agencies can support them.
    12. Coordinate and communicate with clients, service providers, and family members to ensure optimal utilization of services and adequate provision of needed care.
    13. Seek and pick up furniture for newly housed youth.

2.     Complete follow-up tasks to help youth achieve their goals, which includes:

a.      Following up with youth by phone/email/agency maintained social media.

b.     Entering youth data into REC’s electronic record and work with the Housing team to continually improve systems to track and transmit youth outputs and outcomes.

3.     Develop expertise in scattered-site supportive housing and the specific case management components it requires, such as providing:

a.      Regular case management to youth in the community, at youth’s home, job and other locations.

b.     Intensive, youth-centered budgeting and financial goal setting.

c.      Workforce and education supports as identified by youth and guided by REC’s Talent Development Director.

d.     Navigating rent court, specifically cases regarding escrow and eviction.

e.      Learning landlord and tenant rights and helping youth advocate for themselves.

    1. Provide a range of supportive Behavioral Health and Substance Use Disorder services including case management, specialized support (example: tobacco cessation services, certified peer support services, etc.), and consultations to support strength-based care.

g.     Assist clients in transition and discharge planning, aftercare planning, and coordination with external supports to ensure continuity of care.

h.     Model and teach clients life skills and healthy coping mechanisms.

i.       Develop and maintain a friendly network of referrals in the health care and social services community by becoming familiar with formal and informal services available to clients and families, reporting new services, changes in established services, and identifying gaps in available community services.

    1. Maintain a cooperative working relationship with other staff, physicians, other professionals, and the public by facilitating and enhancing communication, and by displaying honesty, respect, sensitivity, and compassion.

 

NOTE: This position will be community-based (with a focus on youth-served at scattered sites). Transportation and mobile travel is required.

WORK ENVIRONMENT:

This job is primarily a community-based position, which includes meeting with clients one-on-one in their homes, at the Ruth Ellis Center, or other community settings (libraries, fast food restaurants, parks, partner agencies, hospitals, etc.) with some exposure to communicable disease. This role routinely uses standard office equipment such as computers, phone, photocopiers, filing cabinets, and fax machines. The noise level in the work environment and job sites may be loud. During the COVID-19 pandemic the Ruth Ellis Center seeks to provide a flexible work schedule to allow remote work and the provision of telehealth services, when appropriate and requested or approved by clients, to promote social distancing.

This position requires training, completion, maintenance, and execution of the full Handle with Care technique. This involves verbal de-escalation and non-violent prevention based holds for individuals self-harming or harming others.

This is a full time, permanent position. The position has some flexibility in scheduling for meetings and other requirments. Some evening and weekend hours may be required to meet the duties of this position. The schedule is generally as follows:

TRAVEL:

This position requires local travel to complete errands and duties as assigned. In-state and national travel may be required up to 5% of the time. The employee must have a valid driver license approved for airline travel.

 

REQUIRED EDUCATION AND EXPERIENCE:

1.     Two (2) years’ experience in victim of crime services (VOCA), homelessness recovery services, HIV services, substance use disorder, behavioral/mental health, and/or medical field.

2.     Ability to take a person-centered approach, show empathy, establish clear boundaries, and promote empowerment.

3.     Ability to de-escalate crisis situations and assist individuals in crisis achieve safety.

4.     Ability to meet deadlines and manage multiple ongoing tasks. Ability to prioritize competing demands for time and attention to ensure clients can achieve their goals.

5.     Experience with Microsoft Office and Google Suite Software.

 

PREFERRED EDUCATION AND EXPERIENCE:

1.     Licensed Bachelor's Social Worker (LBSW); Limited License Bachelor's Social Worker (LLBSW); Bachelors in a Human Services Field.

2.     Credentialed as a Certified Peer Support Specialist (CPSS), Qualified Mental Health Professional (QMHP), or Child Mental Health Professional (CMHP) OR ability to obtain such credentialing. CPSS credentialing involves a lived experience and journey in receiving public mental health services and supports, along with completion of a required training through the Michigan Department of Health and Human Services. QMHP and CMHP credentialing requires specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience) OR one year of experience in treating or working with a person who has mental illness; AND is a psychologist, physician, educator with a degree in education from an accredited program, social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, behavior analyst, registered nurse, therapeutic recreation specialist, licensed/limited-licensed professional counselor, licensed or limited licensed marriage and family therapist, a licensed physician’s assistant, OR a human services professional with at least a bachelor’s degree in a human services field.

3.     Experience with the homelessness services system, including working with the Continuum of Care and Coordinated Assessment/Entry and utilization of the Homeless Management Information System (HMIS).

4.     Experience with Detroit Wayne Integrated Health Network billing and services (formerly Detroit Wayne Mental Health Authority) OR billing and services with another Community Mental Health Authority.

5.     Experience with Social Security Administration, Department of Health and Human Services, or with a Community Assistance Program.

  1. SOAR (SSI/SSDI Outreach, Access, and Recovery) trained or completion within a mutually agreed to timeframe.

7.     LGBTQ (Lesbian, Gay, Bisexual, Transgender, and Questioning) cultural and diversity competency.

8.     Familiarity with the Ruth Ellis Center’s principals of work (trauma informed care, harm reduction, restorative justice, and positive youth development).

9.     Familiarity with principles of strengths-based case management, including Motivational Interviewing techniques.

 

WORK AUTHORIZATION / SECURITY CLEARANCE:

  1. Must be able to pass a criminal background check as outlined by agency policy.
  2. Have no record on the Central Registry for child abuse or neglect in the employee’s state of residency or employment.
  3. Be able to produce a non-reactive tuberculosis test from within the previous twelve (12) months.
