A multidisciplinary Substance Abuse Agency specializing in medication assisted treatment has an opening for a full-time Substance Abuse Counselors in both the Bridgeport and New Haven facilities. The candidate will provide motivational and educational counseling to persons suffering from addiction illnesses.

Key responsibilities include but are not limited to:


1.

Conducts individual counseling sessions, opened groups, closed groups, family counseling sessions.

2.

Presents alcoholism and substance abuse educational information to patients and significant others.

3.

Functions as part of an inter-disciplinary team as case manager. Ensures referrals to modalities of such as determined by the needs of the client.

4.

Write clear, timely, concise progress notes, document areas of conflict, short and long term treatment planning, psych/social history and discharge summaries.

5.

Assessment of new clients upon admission.

6.

Conducts small group activities.

7.
Attends case reviews.


Requirements
Skills/Experience:



Associates Degree or above in Human Services or related field; CAC/CADC certified, LADC/LCSW or Experience with treating opiate addicted clients using Medication Assisted Treatment preferred; Applicants must have knowledge/experience with treatment plans, progress notes, and biopsychosocials.Bilingual a plus; Computer knowledge and basic keyboarding skills a must.
