Boston Nursing Care & Consulting is seeking an experienced and detail-oriented Intake & OASIS Registered Nurse (RN) to join our growing home healthcare team on a PRN basis. This is an in-person role supporting patients and operations throughout Boston and the surrounding communities 🏡. This position is essential in ensuring accurate patient intake, timely admissions, and high-quality OASIS documentation to support compliant and efficient patient care.

• Review and process new patient referrals and intake documentation
• Coordinate start-of-care (SOC) visits and admissions across the Boston area
• Complete and ensure accuracy of OASIS assessments (SOC, ROC, Recertifications, Discharges) in the field
• Ensure compliance with Medicare, CHAP, and agency documentation standards
• Collaborate with field clinicians and office staff to ensure timely and accurate documentation 🤝
• Perform quality assurance (QA) audits on OASIS documentation as needed
• Communicate effectively with referral sources, physicians, and internal team members 📞
• Travel locally to patient homes as required 🚗

• Active Registered Nurse (RN) license in Massachusetts
• Required: Proven experience with OASIS documentation
• Preferred: Experience with Alora EMR 💻
• Minimum of 1–2 years of home health experience
• Strong knowledge of Medicare guidelines and home health regulations
• Excellent attention to detail and organizational skills
• Ability to work independently and manage tasks efficiently
• Reliable vehicle🚘

• OASIS-E knowledge and competency

• Ability to manage both fieldwork and intake responsibilities
