Job Summary: 

Under general supervision, ensures the consumer is able to live independently as possible in the community by determining each individual’s needs to achieve independent living status. Advocates on behalf of the consumer for their unique individual needs.

 Job Duties:

  • As a representative of CDR, be committed in all that you do to support, perpetuate, and model the CDR Mission. 

  • Receives calls and inquiries about the program and advocates the advantages and positives of the program and what the program entails.

  • Screen calls to determine if information given is adequate to open a case or to further assist or monitor consumer once the case has been opened and worked on.

  • Researches appropriate housing, securing transportation, and other needs as applicable for each consumer.

  • Advocates for benefits, services, doctors, or therapists as needed for each consumer.

  • Listens and correctly determines the root of the problem prioritizes problem and systematically resolves any issue(s) that may arise while the case is open.

  • Determines correct services needed for the individual and amount of advocacy needed for the individual consumer and how much to intercede.

  • Decides the appropriate length of time for providing services to the client and what to refer out of the program.

  • Follows up on behalf of consumer on benefits, medical appointments, prescriptions, etc.

  • Recording of daily case notes into the proper computer system database.

  • Weekly: Updates on case notes, Staff meetings.

  • Monthly reports on: Hours served in the community and outside positive direct contact with the media promoting the program to Coordinator of Independent Living.

  • Rotation of department beeper amongst ILS staff and appropriate response to situations as they occur.

  • Other duties as assigned or required by the Coordinator of Independent Living.

     Qualifications:

  • Proficient written and oral language skills, bi-lingual in English and Spanish is a plus.

  • Minimum Associate Degree with three (3) to five (5) relevant experience OR

  • Bachelor’s Degree in Psychology, Social Work or related field with at least one (1) experience working with or advocating for persons with disabilities.

  • Education and relevant work experience can be substituted for personal life experience.

     Essential Functions/Physical Requirements:

    The Job Duties are representative of the essential functions of the position. The physical requirements for this position are those typically present in normal office environment conditions. Operational flexibility is required to meet sudden and unpredictable needs. Reliable transportation is necessary as there may be local day business travel required of this position.
