Community Integration Counselor

I.          JOB SUMMARY

This position is responsible for assisting waiver program consumers who are experiencing significant problems managing the emotional responses inherent in adjusting to a significant physical or cognitive disability while living in the community.

II.         DUTIES

Essential Function Duty

Conducts counseling sessions to persons with disabilities and their families. 

Completes psychosocial assessments. 

Develops, implements, and monitors detailed goal plans. 

Conducts six (6) month service reviews and complete Individual Service Reports (ISR). 

Completes progression notation for each direct counseling session. 

Initiates and maintains client charts per Department of Health (DOH) regulations. 

Initiates SRI accounts as needed. 

Performs service specific intakes including intake paperwork. 

Performs other duties as assigned. 

III.        SUPERVISION

This position does not have any regular responsibility for the work of others.

This position may require local travel. 

VI.       WORK ENVIRONMENT

This job operates in a professional office environment with moderate to light noise. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

VIII.     Qualifications

Education Requirements

Master of Social Work, Doctorate of Psychology, Counseling of Psychology or related field but will accept a License in Psychiatry, Psychology, Mental Health Practitioner or Certified Rehabilitation Counselor, Certified Education Teacher

Experience Requirements

Two (2) years of experience providing adjustment related counseling to individuals with traumatic brain injuries and their families

Skills and Competencies Requirements

Verbal and written communication skills

Customer/consumer focus
