Job Summary:

With general supervision, provide information, assistance, and advocacy to individuals, family members, and their caregivers seeking Long Term Services and Supports (LTSS). Work in conjunction with the designated NY Connects sites to fulfill program requirements including providing direct services to individuals, family members, and their caregivers, participating in local Long Term Care Council meetings, and conducting outreach on accessing LTSS.This is a 15 month grant funded program starting January 1, 2017 and ending March 31, 2018

 General Expectations:

Provide barrier-free information and assistance (including home visits if necessary) to enable clients access to LTSS.

  1. Perform information and assistance functions from the NWD Screen for NY Connects including phone calls, walk-ins, and off-site visits.
  2. Follow the NY Connects NWD protocols to provide application assistance including submitting completed applications, determining eligibility, and provide linkage and referral for necessary supports.
  3. Work with the local NY Connects program to conduct outreach and education about enhanced LTSS including Consumer Directed Personal Assistance Services (CDPAS), Personal Care Services (PCS), Durable Medical Equipment (DME), Waiver Services, Healthcare Advocacy, Benefits Advisement, and other enhanced LTSS.
  4. Maintain regular presence at a designated county NY Connects site as part of RCIL’s service area through this program (Monroe, Wayne, Ontario and Yates County).
  5. Collect prescribed information on individual, family member, and caregiver needs including financial and program eligibility as well as types of services and resources that might best meet their needs.
  6. Facilitate safe transitions for individuals transitioning from institutional to home and community-based settings.
  7. Provide trainings on Public Assistance benefits, SSI/SSDI benefits, Medicaid and Medicare, Mainstream Managed Care and Managed Long Term Care.
  8. Work with NY Connects NWD to implement the State evaluation plan for consumer, program, and systems. 
  9. Participate in local Long Term Care Council meetings.
  10. Submit in a timely manner and maintain documentation and case notes in online database, CSR, and paper files.
  11. Carry out assignments from the Lead NWD IL Specialist and  IL Supervisor. 

Qualifications:

High school diploma required. Associates degree preferred in addition to two years of experience in the Human Services field. Knowledge and experience accessing and advocating for LTSS such as CDPAS, PCS, DME, and waiver services, through Medicaid and other community-based services. Knowledge and experience with benefits programs including the appeal processes for Medicaid, Temporary Assistance, Supplemental Nutrition Assistance Program, Supplemental Security Income, and Social Security Disability Insurance. Essential Functions/Physical Requirements:

The duties are representative of the essential functions of the position. The physical requirements for this position are those typically present in normal office environment conditions. Operational flexibility is required to meet sudden and unpredictable needs. Travel throughout RCIL service area through this program which includes Monroe, Wayne, Ontario,  and Yates County, is required.
