Job Description:
As a Registered Nurse specializing in home care, you will play a key role in providing comfort, support, and skilled nursing care to patients in hospice. You will work closely with an interdisciplinary team to ensure that patients' physical, emotional, and spiritual needs are met during their final stages of life.
Key Responsibilities:
- Provide direct patient care in accordance with the patient's plan of care
- Administer medications and treatments as prescribed.
- Monitor patients’ symptoms and report changes in their condition to the hospice team.
- Assist with pain management and provide emotional support to patients and their families.
- Collaborate with the hospice team to ensure comprehensive care, including coordination of services such as social work and chaplain support.
- Maintain accurate and up-to-date medical records.
- Provide patient and family education on hospice care and what to expect in the end-of-life process.
- Ensure patient comfort and dignity, addressing any physical or emotional distress.
- Participate in interdisciplinary team meetings to review patient progress and adjust care plans as needed.
Qualifications:
- Active and unrestricted Registered Nurse license in the state of Ohio.
- Minimum of 1 year of experience in home health, or a related field preferred.
- Compassionate and patient-centered approach to care.
- Strong communication and interpersonal skills.
- Ability to work collaboratively with an interdisciplinary team.
- CPR certification required.
- Ability to handle emotional situations with empathy and professionalism.
- Reliable transportation and willingness to travel to patients' homes or hospice facilities as required.
