I.          JOB SUMMARY

This position provides advocacy, support, linkage, and coordination of services for an assigned caseload.  This position develops individual service plans in conjunction with assigned caseload. This position also participates in team meetings with participants and their service providers, and advocates for their rights and implementation of needed and desired services.

 II.         DUTIES

Essential Function Duty  

Maintain files that are current and in compliance with the company and State regulating agency. 

Ensure that all application incident reporting requirements are followed. 

Meets all documentation timelines and maintains quality of services. 

Participate in trainings at the discretion of the supervisor and strives for professional development and to increase knowledge base related to participant population. 

Review and implement all regulatory changes to programs. 

Develop and revise the participant’s service plan in conjunction with the service provider they choose to receive services from. 

Link the participants with an appropriate service provider who can assist them in implementing the appropriate portion of their service plan. 

Assist the participant in securing a community residency that meets their individual welfare and safety needs. 

Review the outcomes of the participant’s service plan and advocate for the needs and changes in the services as necessary in combination with the participants needs and desired outcomes. 

Schedule and lead participant team meetings. 

Maintain communication with participant service providers and families. 

Conduct monthly face to face visits with assigned case load participants. 

Maintain records, plans and case notes on a daily basis for consumers. 

Attend supervisions. 

Perform other duties as assigned. 

III.        SUPERVISION

This position does not have any regular responsibility for the work of others.

This position requires frequent travel.

VI.       WORK ENVIRONMENT

This job operates in a professional office environment with moderate to light noise. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

VIII.     Qualifications

Education Requirements

A minimum of an Associate’s degree in Human Services with three (3) years of Case Management experience providing service coordination for individuals with disabilities and/or seniors and knowledge about community resources;

Bachelor’s degree in Human Services and two (2) years of case management experience; -or-

Graduate degree plus one year of case management experience providing service coordination.

Experience Requirements   

Skills and Competencies Requirements 

Possess a basic knowledge and understanding of public welfare, Social Security benefits and entitlements. 

General knowledge and understanding of developmental disabilities, physical disabilities, traumatic brain injury, geriatrics and mental illness.

Having knowledge and understanding of the challenges facing individuals who have an developmental, physical and cognitive disabilities, and seniors who choose to live independently in the community.

Basic knowledge of the Americans with Disabilities Act, and a desire to learn more about the public laws that uphold the civil rights of disabled individuals.

Some knowledge of community services available to the citizens of the community.
