A Sign-On Bonus of $3,500 is being offered for qualified individuals hired.


The ideal candidate will be responsible for providing counseling, education and case management services to chemically dependent individuals. 

Qualifications:
Master's degree with NH MLADC or LADC Certification/License preferred; previous experience working with chemically dependent individuals, must demonstrate a positive non-judgmental and accepting attitude toward the chemically dependent patient.


Responsibilities:

Patient Care:

  1. Conducts individual initial assessment and evaluation interviews for service eligibility determination, including urine specimen collection.
  2. Facilitates, establishes and reviews with each patient the initial treatment plan
  3. Completes Bio-psychosocial assessment w/in 30 days.
  4. Conduct a monthly treatment plan review for patients accepted into the medically supervised withdrawal program.
  5. Conduct a quarterly treatment plan review for patients accepted into the maintenance program.  
  6. Provide 1 hour of individual or group counseling services every 30 days, at regularly scheduled intervals, coordinating all services throughout treatment.
  7. For patients admitted for supervised withdrawal services 2 hours of individual counseling will be provided every 30 days.
  8. Involve families and significant others in patient’s recovery process whenever possible.
  9. Develops and participates in individual or group patient teaching projects.
  10. Respond, as needed, to patient grievances and complaints.
  11. Develop aftercare plans.
  12. Develops discharge plans.
  13. Completes all discharge paperwork and closes out active chart w/in established timeframe.
  14. Other related duties as determined by supervisor.

 

Administrative:

  1. Participates in department meetings, committees and quality improvement initiatives.           
  2. Participates in weekly case review with team members.
  3. Assist in quality assurance chart reviews, assessing clinical and medical documentation to ensure compliance with regulation and program standards.
  4. Meets all billing and paperwork requirements.
  5. Maintains prompt and courteous relationships with Managed Care Providers insuring continued coverage, completes all required paperwork w/in stated deadlines.
  6. Participates in teaching of other nursing and ancillary personnel and students.
  7. Establish discharge linkages.
  8. Cooperate with all program staff and other community agencies.
  9. Maintains current knowledge of developments in the substance abuse treatment field and is responsible for self-development through continuing education programs.
